Overview
Life and AD&D (accidental death & dismemberment) insurance offers another layer of financial protection for you and your family. Benefits are paid if you or a family member dies or experiences a covered accident.

How the Plans Work

AEO pays for your basic coverage
AEO automatically enrolls you in Basic Life and Basic AD&D coverage and pays 100% of the cost.

You can elect supplemental coverage
For extra protection, you can elect Supplemental Life for yourself, your spouse/domestic partner and your children. You can also elect Voluntary AD&D for yourself.
You pay the full cost of any additional insurance you purchase through an after-tax payroll deduction.

Choose a beneficiary!
Make sure you choose a beneficiary for each type of coverage elected. A beneficiary is the person who receives the benefits if the covered individual dies. You can complete or update your beneficiary designations on the Benefitfocus portal.
Evidence of Insurability (EOI)
EOI is a statement of health that insurance companies may require before your life insurance is effective. You’ll need to complete this form from Aflac if:
- You elect coverage of 3x-5x your annual base salary.
- Your spouse/domestic partner elects coverage of more than $40,000 or an increase of more than one benefit level.
- You did not enroll when you were first eligible.

*Unmarried dependent children from birth to 26 years old are eligible for coverage. When you elect child life, all of your eligible children are covered.
Costs for Coverage
You’ll see your rate for supplemental coverage when you log in to the enrollment system. Rates for yourself and your spouse/domestic partner will vary based on age. An age reduction schedule applies starting at age 70.
FAQs
What happens if I need to file a claim on my life or AD&D insurance?
If you or a family member experiences a covered injury or loss, contact Aflac at 844-440-1063. Aflac will review the claim and, if approved, pay out benefits to the beneficiary on file (or to you, in the event of a covered AD&D injury).
Do I need to enroll in life and AD&D benefits each year?
No, once you are enrolled in the benefit, you will stay enrolled unless you make a change or you become ineligible for AEO benefits.
